New Patient Registration
Colinton Medical Practice can only accept patients living within our practice boundary.
Unfortunately Colinton Medical Practice is declared ‘full’ which means that it is currently only able to register those new patients who are moving into the area, newborn to a family already registered, or asked to leave another Practice because you are outside that Practice’s boundary. If you are unclear whether you meet the criteria to register please enquire at Reception. Temporary Residents to the area can be treated by the surgery.
Documentation Required For Registration
If you are new to the area and wish to register with our practice please bring along the following documentation before 1700:
Proof of address and of identification (photographic).
You will be asked to show photographic proof of Identification (ideally this should be your passport if you possess one) and proof of address, for example a utility bill, (or, if you have just moved to the area, proof of house purchase or rental). If you are visiting from overseas you will need to show your passport and certain other documentation may be required dependent upon your country of origin , the Reception Team will advise on the specific requirements.
When registering you will also be asked to complete our New Patient Registration forms and a Questionnaire as these allows the doctor to familiarise him/herself with your medical history and healthcare needs.
You may download these forms via the links below BUT THESE MUST BE BROUGHT PERSONALLY INTO RECEPTION BEFORE YOU CAN REGISTER AS YOU ALSO NEED TO BRING PROOF OF ADDRESS (eg mortgage or rental agreement) AND ANSWER SOME RELEVANT QUESTIONS (please don’t send them in – there are questions the receptionist needs to ask and she needs to see the proof of address and identification).
These forms are all available at reception.
Appointments and Medication After Registering
- Appointments: Unfortunately we cannot register you and immediately give you an appointment as the registration process takes at least 24 hours. Therefore please allow 24 hours between registering and asking for your first appointment.
- Medications: If you are on any medications on joining us, you will need to make an appointment with a GP once registered before any medication can be prescribed by our GPs.
Change of Details Form
Please ensure that you enter a mobile phone number and if you change your name, address or telephone number, please let us know. We will then update our records. Download our change of details form via the link below, complete and bring to the Medical Practice. Please bring this personally into Reception with proof of the change of details .

Change of Address
If you move house you must let us know. If you are moving to a new address within our practice area please complete and hand in a Change of Address Form. PLEASE BRING IN THIS PERSONALLY TO RECEPTION WITH PROOF OF THE NEW ADDRESS (eg mortgage/rental agreement).
If your new address is outside our practice you will have to register with a practice that covers your address. It is strongly advised that you do this quickly on moving home as we are unable to provide house calls and any other out of area service. Change of Name/Phone Number etc.
If you change any details please keep us informed in case we have to contact you for any reason. Just contact reception.
